A man in his 40s has been left in a critical condition after being crushed by machinery at a Perth factory on Monday afternoon. Emergency crews rushed to the site as investigations began into yet another serious workplace accident at the same family-run business.

Details of the Incident
Emergency services were called to Hoffman Engineering in Bassendean, in Perth’s north-east, around 2:40pm on September 8, 2025. The worker was crushed by factory machinery, leaving him with life-threatening injuries.

St John Ambulance confirmed that no patient had been transported to hospital immediately, with emergency crews remaining at the site well into the evening.

History of Safety Breaches
This is not the first serious incident at the Bassendean facility. Six years earlier, in 2019, a 46-year-old father was killed after being crushed by machinery weighing more than a tonne.

The company was subsequently prosecuted and fined for failing to provide a safe workplace over that fatality.

The recurrence of another crushing incident at the same premises has raised questions about the adequacy of workplace safety improvements implemented since then.

Regulatory Response
WorkSafe WA has been called in to investigate the circumstances of Monday’s accident. Inspectors are expected to examine machinery maintenance records, worker training, and the safety protocols in place at the time of the incident.
