Introduction

In February 2023, a tragic incident occurred at a building material supply workplace where a forklift operator was fatally injured after being struck by a chain. The accident took place during the process of unloading steel weld mesh from a shipping container.

Details of the Incident

Preliminary investigations suggest that the forklift was using a connection point in its counterbalance to drag material out of a container. During this operation, it appears that part of the towing equipment failed, causing the chain to recoil and strike the operator.

Operational Context

The incident underlines the risks associated with material handling operations, especially when using forklifts for complex tasks like unloading heavy materials from shipping containers. The failure of the towing equipment points to potential issues in equipment maintenance and safety checks.
