Fatal Workplace Accident at Woorinen South Leads to Charges Against Two Companies

In January 2022, a 70-year-old employee fell from a trailer attached to a tractor near Swan Hill in Woorinen South, Victoria, sustaining fatal head injuries. The worker was employed by contractor AH Vision Pty Ltd and was working on behalf of Cutri Fruit Pty Ltd at the time of the incident.

WorkSafe Victoria has since laid charges against both companies, alleging failures in safe work systems, training, and information provision. The case is scheduled for a filing hearing at Swan Hill Magistrates Court.

WorkSafe Victoria has since laid charges against both companies, alleging failures in safe work systems, training, and information provision. The case is scheduled for a filing hearing at Swan Hill Magistrates Court.

WorkSafe Victoria investigated the incident and determined that both companies had breached their obligations under the Occupational Health and Safety Act. Cutri Fruit Pty Ltd faces five charges for failing to provide a safe system of work and necessary information and training to workers, and for not ensuring the safety of individuals other than employees. AH Vision Pty Ltd is charged with failing to provide adequate instruction and training to employees, and with failing to provide WorkSafe with required information and documents.
