Luke O’Brien was working in the fuel bay area of the Saraji mine when an interaction with two vehicles resulted in him being fatally crushed. The young man suffered significant crush injuries after being trapped between two vehicles in the fuel-bay area of the mine on Monday afternoon. Despite being taken to the hospital, he succumbed to his injuries.

Resources Safety and Health Queensland (RSHQ) has launched a full investigation into the incident, with multiple investigators and inspectors examining the site. The police are also involved, preparing a report for the coroner.
