Introduction
Authorities are investigating the death of an 80-year-old female passenger who was discovered deceased on Lizard Island during a shore excursion from Coral Adventurer, a vessel operated by Coral Expeditions. The incident occurred on October 25, 2025, while the ship was visiting the remote island roughly 320 km (200 mi) north of Cairns, Queensland.

Details of the Incident

Excursion and Disappearance:

  • The passenger joined a guided hike to Cook’s Look, the highest point on Lizard Island.
  • Partway through the climb, she informed the group that she was unable to continue and would return to the ship alone.
  • Witnesses later noted that Coral Adventurer departed without realizing the passenger had not reboarded.
  • When her absence was discovered later that evening, a search effort was initiated both onboard and ashore.
  • Her body was found the following day.

Investigation and Regulatory Oversight:

  • The case has been referred to the Queensland Coroners Court for formal investigation.
  • The Australian Maritime Safety Authority (AMSA) confirmed it is reviewing compliance with passenger-tracking procedures required under maritime safety regulations.
  • An AMSA spokesperson stated that investigators will assess whether proper headcounts and excursion oversight protocols were implemented and will take action if non-compliance is identified.
  • AMSA also intends to board the vessel when it arrives in Darwin to conduct additional inquiries.

Industry and Expert Commentary:

  • Marine scientist Dr Adam Smith of James Cook University observed that while at-sea operations typically include strict headcounts and supervision, land excursions sometimes rely on self-reporting.
  • He suggested stronger safeguards such as “buddy systems” and formal checklists for onshore tours to prevent similar tragedies.
  • Gareth Phillips, CEO of the Association of Marine Park Tourism Operators, emphasized that all commercial passenger operators are legally obligated to maintain rigorous safety management and verification systems.
  • He described Coral Expeditions as a long-standing operator with a strong safety record and stated that the industry remains committed to the highest safety standards.

Company Statement:

  • Mark Fifield, CEO of Coral Expeditions, confirmed the company is fully cooperating with authorities and expressed condolences to the passenger’s family.
  • The company’s care team is supporting both the family and the passengers and crew affected by the incident.
  • Coral Adventurer resumed its itinerary after clearance from authorities; it was completing the first leg of a 60-day circumnavigation of Australia at the time.
