Circumstances:

On July 20, 2025, a Reims-Cessna F406 Caravan II aircraft registered VH-EYQ crashed into a field approximately 1.4 nautical miles from runway 14 at Oakey Airport, near Toowoomba, Queensland. The twin-engine turboprop was destroyed on impact, and both occupants were fatally injured.
The aircraft had departed Warwick Airport at 14:26 and was reportedly conducting a routine proficiency check, commonly undertaken by pilots to assess their ability to conduct instrument-based flight manoeuvres. During the approach to Oakey, the aircraft issued a request to land before crashing into a grassy paddock near Oakey Cooyar Road in Kelvinhaugh.

Eyewitnesses reported hearing a loud explosion, followed by a large fireball upon impact. Queensland Fire and Emergency Services responded to extinguish the post-crash blaze. The Queensland Police confirmed the aircraft had caught fire upon impact, and that both individuals onboard died at the scene.

The ATSB has commenced an investigation and plans to recover the wreckage for detailed examination at its technical facilities in Canberra. Authorities have appealed to the public for any video footage of the aircraft before or after the crash.

The incident remains under investigation with forensic identification of the deceased underway. The Forensic Crash Unit and multiple partner agencies are assisting in determining the cause of the accident.
