Incident Overview
A Queensland passenger train struck the rear trailer of a stranded B-double at the Bonemill Road level crossing in Runcorn on 14 August 2025.
People and Injuries
The train carried about 300 people. Six passengers received treatment for minor injuries; the train driver, guard and truck driver were not physically injured.
Mechanical Failure
The ATSB found a sudden air-line failure between the truck’s trailers left the rear trailer blocking the tracks.
Warning Sequence
The trailer remained across the crossing for more than four minutes before the lights and boom gates activated.
Emergency Reporting
The ATSB found neither the truck driver nor witnesses called the railway emergency number or triple zero, so network control was not alerted to the obstruction.
Severity Factors
Investigators said the train driver’s emergency braking and the trailer being unloaded and stationary likely reduced the severity.
Safety Actions
The passenger rail operator said it would review level-crossing management and work with the heavy-vehicle regulator on safety education.

The ATSB said anyone whose vehicle becomes immobilised on railway tracks should move clear and immediately report the emergency.
