Incident Overview
A 74-year-old StarTrack postal worker has died following a shocking crash that sent two vehicles through an office building and into an underground carpark in West Perth on Monday morning, October 27, 2025.

The crash occurred around 6:40 am at the intersection of the Mitchell Freeway off-ramp and Hay Street, involving a Hyundai Tucson and an Isuzu D-Max.

The female driver of the Hyundai, a long-serving StarTrack employee, sustained critical injuries and was pronounced deceased at the scene. The other driver, a 38-year-old man, suffered minor injuries and was transported to Royal Perth Hospital for treatment.

The impact caused both vehicles to plough through the front of a commercial building before falling partially into its underground carpark. Specialist fire crews stabilised the scene to prevent further collapse while cranes were brought in to remove the vehicles.

  • Eyewitness accounts and dashcam footage from vehicles travelling near the Hay Street and George Street intersection between 6:30 am and 6:50 am.
