Incident Overview
On Monday, 8 September 2025, a man in his 40s was critically injured after being crushed by machinery at Hoffman Engineering in Bassendean, Perth’s north-east. Emergency services attended the site around 2:40pm local time, and the worker remained at the facility under emergency care well into the evening with life-threatening injuries.
WorkSafe WA has been notified and has commenced an investigation into the circumstances of the incident. The accident marks the second serious crushing incident at the same family-run business in six years.
