Incident Overview
The Queensland Police Forensic Crash Unit is investigating a head-on collision between two utility vehicles at Westbrook on Sunday, 13 September 2026. A 28-year-old woman died at the scene and a 22-year-old man was taken to hospital with minor injuries.
Police said initial investigations indicate a black Mazda B2500 was travelling on Toowoomba Millmerran Road at about 6.06am when it collided head-on with a black Nissan Navara.

Vehicles and People Involved
- A black Mazda B2500 driven by a 28-year-old Southbrook woman, who was the vehicle’s sole occupant.
- A black Nissan Navara driven by a 22-year-old Pittsworth man, who was also the sole occupant.
Fatality and Injuries
The Mazda driver was declared deceased at the scene. The Nissan driver was transported to Toowoomba Base Hospital with injuries police described as minor.
Police Investigation and Appeal
Forensic Crash Unit investigations remain ongoing. Police have asked anyone who witnessed the collision, or who has relevant information or dashcam footage of either vehicle, to contact them.
